1. PLEASE ENSURE YOU SELECT THE CORRECT SCHOOL FOR 2022-2023 SCHOOL YEAR FOR YOUR STUDENT ATHLETE.

2. DO NOT USE ATHLETE NICKNAMES ON FORMS. USE FULL AND COMPLETE NAMES.

3. You MUST have your athlete’s school ID number available as it is required on each form.

*To access the online forms, hold down your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of the forms.

*Click on the form name and fill out the information requested. To sign the document, click inside the signature box and hold your mouse down. This will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box.

*Once you have filled out all the information on each page, you will have the opportunity to print the document.

*Even if you are 18 or older parents still must fill out these forms.

*You will receive a confirmation email once the document has been reviewed by the Liberty ISD staff.

• Note for physicals: All physicals should be completed on the current up-to-date UIL Pre-participation Medical History and Physical Form for 2022-2023 school year not prior to April 1, 2022. You may click tab above for the PDF form to be printed and taken to the physician of your choice. The front page MUST be completed and physically signed by both the parent/guardian and the student athlete in order to be eligible to participate in athletics.

**All high school and middle school athletes must turn in their physical forms to the coach of your son/daughters sport at their campus or the athletic trainer. Physicals must be on file for athletes to participate in a practice session for all 7-12 grade students.
